The Success Sequence Program SRAE Site Agreement

Instructions for completing this form: Please review, approve, and sign the Site Agreement below. Next, please complete an implementation plan for each class to receive programming at this site. The information to be completed below will allow the assigned program staff to easily enter your school/site implementation plans into the required data system.

  • Site Agreement for Paper/Workbooks or Digital/Online

    This document provides terms and conditions for utilization of the Success Sequence Program (SSP) in a school orother program setting. This agreement may be terminated upon request.

    • We have reviewed the Success Sequence Program (SSP) materials, Game Plan, Quest, Aspire and/or Navigator and deemed the program(s) appropriate for implementation.
    • At least eight (8) classroom periods will be dedicated to implement the program with fidelity along with any necessary support services.
    • Performance measures documents, including student enrollments and attendance will be collected and provide to A&M Resources within 2 weeks of completing the program.
    • As part of a federally funded program, youth will be asked to complete voluntary, confidential entrance and exit surveys.
    • Students will be asked to complete a voluntary, confidential evaluation survey before and after programming to help measure the effectiveness of the program, which may include a 3-month follow-up survey.
    • The SSP promotes optimal health and future success by encouraging youth to embrace positive youth development and avoid risk behaviors.
    • The use of the Success Sequence Program materials does not hold A&M Resources liable for any legal claims against your school or agency.
    • Educator observations will be permitted to ensure program fidelity.
    • Educator will follow mandated reporting procedures as required by law.

    A&M Resources agrees to:

    • Help identify support services as needed for all youth and their families.
    • Ensure program materials and activities are age and culturally appropriate, trauma informed, and medically accurate.
    • Collect and store participant demographic information and other reporting requirements in a secure database under restricted access policies.
